The following Request Form is an online form needed to reserve, promote, and add your event to the church calendar.
This REQUEST FORM must be completed and turned into the church office for consideration for use of any LSBC property or resources, including but not limited to Events, New Groups, Rental Use of the property, etc.
PLEASE NOTE: This Form MUST be completed and turned into church offices NO LATER THAN 4 WEEKS prior to your event start date for your request to be considered. All submissions are considered and you will be notified if it is approved.
